If you are concerned that you (or a friend or relative) may be developing Alzheimer’s disease or other dementia, you are urged to discuss this with your primary care physician. He or she may want to refer you to a dementia specialist, such as a neurologist, neuropsychiatrist, geriatrician, or neuropsychologist.

However, we are able to determine whether an older person is at increased risk for having dementia based on your responses to a few questions. This Dementia Risk Assessment is NOT meant to replace in-person evaluation by a qualified physician or other dementia specialist. The information provided here is intended for educational purposes only, and to help guide your healthcare decision-making.

This is the second edition of the Memory Survey and was released on November 1, 2014. You are eligible to take this assessment even if you previously took the first edition.

Also, you should ask someone who has known you extremely well for many years (e.g., spouse, adult child, very close friend) to visit this website and click on the “Take Assessment for a Friend or Relative” button. He or she should enter the same I.D. number assigned to you (see your print out).
